I understand and acknowledge that participating in this 5K event, whether as a runner, walker, or participant in any other capacity, involves inherent risks, including but not limited to physical exertion, falls, contact with other participants, weather conditions, road or trail conditions, and other hazards that may result in serious injury, illness, disability, or death.

I voluntarily assume all risks associated with my participation. To the fullest extent permitted by applicable law, I release, waive, discharge, and agree to hold harmless Open Heavens Church (OHC), the City of Dallas, the event organizers, sponsors, volunteers, contractors, and their respective officers, directors, employees, agents, and representatives from any and all claims, liabilities, demands, damages, losses, costs, or expenses arising out of or related to my participation in the event, including any injury, illness, property damage, or death, except to the extent caused by gross negligence or willful misconduct where such liability cannot be waived under applicable law.

By registering for and participating in this event, I certify that I am physically able to participate and have consulted a medical professional if I have any concerns regarding my fitness to participate.”
